Japanese phone number format

I can't find the answer to this readily on Google, so hoping someone here might know...

I'm working on a sign-up form for Japan, and the requirement is to break the phone number into 3 fields. I'd like to name the fields something more constructive than 'part1', 'part2', and 'part3', but have no idea what the respective parts are - and nor do the requirements people, it seems.

Does anyone here have any idea?

From the requirements doc, the minimum & maximum lengths for parts 1-3 are 2-4-4 and 6-4-4 respectively, if that helps.

Phone: 011-81-3-3224-5000 Is the number for the US embassy in Tokyo so you have an example of a real world number.

011 is the international dialing
81 is Japans country code
3 is the City of Tokyo code
3224-5000 is the individual number

Not sure how you are going to break that down into 3 sections

the code to identify the phone number does not normally identify the type of long distance you are using or even the country code (most people in the US would be hard pressed to tell you the country code for the US) so you shouldn't need to worry about the 011 or the 81 just the 1 to 3 digit city codes and the individual telephone number.

so you can see that the area code/city code and individual number can somewhat shift in sizes but always are 10 digits long in total.
