J169 4.0.0.0.21 wont upgrade

I have Avaya IP Office 11.1, and for some reason, 3 of our 4 x J169 phones will now upgrade past 4.0.0.0.21

The upgraded firmware files are uploaded (FW_S_J169_R4_0_6_0_8.bin) and the upgrade file is pointing to this .bin as well. 1 of them upgraded fine, but was already on a newer version, but the 3 that are on 4.0.0.0.21 will not update.
