J139 Phone randomly logging out

I cannot seem to isolate an issue where J139's on 4.0.10 firmware running on 11.1.2.4. (IP500v2 with Application Server VM)
Some phones will log themselves out. The customer is constantly having to log these back in.
I tried factory defaulting the phone. No help.
Tried replacing the phone, no help.

Anyone run into this issue?
 
I thought I had it resolved upgrading to 11.1.2.4.
Issue came back a few weeks later.
These phones don't pull another firmware upgrade but it just an annoyance. Customer is not happy about it.

I will try rebuilding the extensions and see if that fixes it.
 
You can try to upgrade the phones to 4.0.14 firmware.
