I have 5 tables with 31 items (some yes/no and text). When I add a 6th table with yes/no items and text, my form does not work.

Does this make senses? Is there a limit of the number of tables a form will populate?

Your form is limited to a grantotal of 254 controls and sections.
Ask the help wizard for limitations

Good Morning PHV,

When I asked the wizard for the "Form and report specifications" I did get a list of the specifications but I do not belive my database has reached the max. Any suggestions?

Access keeps a history of all the controls (even the deleted ones) in the form.
You may try to Repair/compact your database in the hope of that will shrink this history list.
