Is it possible to setup a fake lan?

I am wondering if it's possible to emulate a LAN setting. Whereas a buddy of mine and I can play a PC game together by choosing LAN in the menu, instead of having to use gamespy or another form of an internet server. Thanks!

A few more details would help. Are you playing over the internet? in the same room?
