Irrelevant message attached to email chain

One of our users received a message from an external client dated jan 30th who also ccd in 2 other external clients and one other internal employee. When the user clicked on reply or reply all there is a message at the top of the mail box that states this isnt the original message in the conversation, click here to open the message. When the user opens the message it is actually a message that is between the user and her internal colleague that dates back to sept 20th and has nothing at all to do with the email she received.
The user is very concerned about confidentiality and why this has happened, i have removed the option in outlook to include the original message in the replies, but the user is gravely concerned as to why this has happened and is concerned it may have happened before. She has hundreds of emails in her mailbox so maybe a one off or she tagged it accidentally?
Any suggestions about this? as you can imagine we have a worried user who wants to know why, any suggestions?
