Ipoffice Conference bridge has no audio

We have an Ipoffice Rel 8 with Voicemail Pro.
When users try to conference people together it appears to connect everyone together but no one can hear each other. This happens with both a manual conference and a meet me conference.

When watching System status it shows them connected to confernece ports.

Any ideas what would cause no audio on the conference? There are no problems on regular calls.

What type of phones?
It sounds like routing or a broken conference chip.

It also happens if you call from the outside into the meet me bridge we have setup in VM Pro.

Then i suggest doing a swap of the IP500
