Have IP403 currently running V2.1,
what is the the highest level release I can upgrade to, ie 4.0? and how do I do it?

The highest you can go to is 3.2.59. Version 4.0.x is right out!
If it is a 403DT (i.e. will work with "old style" index phones) then you can upgrade to 3.2.59 but the DT phones will no longer work. If you have DT phones and wish to continue to use them then you are restricted to staying on version 2.1. (there is a specific 3.0 DT release, but it is essentially the same as 2.1 but has a trial licence support)
