IP Phones Reverted To NOUSER

We have an IPO 500 new with 8.0 Software. We installed 10 new end point licenses and two 5610 telephones. After the phone came up and were tested as working three days later they reverted to nouser. What would cause this to happen?
 
logon with *35*extn*logoncode#
If no logon code is set try 0000

Do you know if you have the Hot Desking option set?
Is Force Login clicked?
and a password set in Login Code?

Did the power go off, or the phone switch get rebooted?

This is what would happen if you're using Hot Desking/Force Login and you reboot the IP 500.

Or are you asking how to log in again, to get the name and extension number showing up again?

You may want to clarify your question.
