IP Phone Transfer

I have a DN that appears on 4 phones as a MCR. Intermittently, sometimes when we try to transfer from a call on this line the phone icons becomes grey and we cannot either go back to the call nor transfer it. It just hangs there with the icon. Also, this sometimes happens when we put the call on hold and try to go back to it.

Has anyone seen/heard of this, more importantly, does anyone know the problem and it's fix??
