IP Office causing analog lines to go down?

One month old Essential IP Office 500V2 8.1 (73). Individual analog lines are going down randomly across 2 analog combo cards. Verizon will restore dial tone and things will work for a few hours or days before dial tone goes out again. The lines are actually dying. I'm testing them at the demarc and there is no dial tone. Verizon will restore, but it happens again. I have replaced one of the combo cards, but no fix. Is there something in the system that could cause this? This only started happening after we installed this system. I am going out to replace the system tomorrow, but just wanted to throw this out there to see if anyone else has experienced something similar. Thanks.
 
Don't replace the system just the combo cards to start with.

I have already replaced 1 of the 2. The new one is doing the same exact thing.
 
Then yes I would replace the ip 500 v2 as well seeing you have already replaced the combo card

I was kind of wondering if the automatic impedance match might be sending something to the carrier that is dropping the line. Could this happen?
 
No this would not cause that type of issue. sounds like a bad box

The lines, when plugged in, immediately go off hook, play a dtmf tone, a screech, then another dtmf tone. This would sometimes kill the line. There is a new setting on v2 combo cards, echo cancellation. I turned this off and the problem was fixed.
