IP Agent/softphone second call over rides first

We're having a strange problem that has developed. In IP agent (v7) or IP softphone (v6), in telecommuter mode only, when a user is on a call and a second call comes in, the user can hear the caller talking as well as the ring coming through and it puts the first caller in a phantom hold that can't be recovered. If the user doesn't answer the call, the caller is sent to voicemail and the user has dead air. The user can still see the first call, but cannot recover it. Road Warrior acts like it's supposed to, as do desk phones. I've tried different stations, different telecommuter phone numbers, different networks... it seems to be pbx related. I'm at a loss... Has anyone ever seen this?

CM 5.2.1 on S8730s.
