IP 1230 phones dying on BCM 50 Rel 5.0

I have a problem that has occurred twice. The first time was Sept 2010 and now Mar 2011. I have a BCM 50 with 3 * IP 1230 phones plugged directly into the 3 spare LAN ports on the front of the unit. Each phone has it's own external 48V power supply to power the phone. The problem is that the phones are completely dead, and will not power up at all. This has occurred 2 times now. The power supply unit is not at fault as it works with the replacement phone. I am suspecting mains power supply issues to the building. Has anybody seen this before.
 
Replace the external power units.
There was a batch off these causing the ip1230 to go faulty.
When you replace the phone itwill work for a while before the replacement phone goes dead.
 
Thanks for the info Snowman. Would you happen to know if there is a Avaya Product Notice for the issue and / or the Serial number range that was faulty.
 
I eventually have found the Product Notice. It does not say what exactly the issue is. Anyway I have found that the LAN port on the phones is still active if you plug phone into POE switch. Therefore the faulty PS just kills the power in circuitry not the whole phone. I will use a POE injector power supply to put power + ethernet directly into phone. These are available from Avaya PN# 1151C1
