Investigating an that email went to wrong recipient

Our MD replied to an email from an external email address and the reply went to an internal user. He wasn't in the BCC box of the sent email and we don't have email tracking on. Their names and email addresses aren't similar at all. Does anyone know how this could happen? Or where I can look.
the email headers show it's from the MD and To the external user

i.e. From: robert to:John yet it ended up in charlie's mailbox.

BTW We're using Outlook 2003 and Exchange 2003 on windows server 2003
 
Please ignore, finally got to look at the MDs sent items and charlie was in the BCC field as I suspected - of course this was denied when I asked the question.
