Intuity Audix Tape drive issue

My customer has an old Audix and there was a problem with the backup tape so new tapes were ordered because it was not clear if the tape was bad- probably the original from 12 years ago. The drive destroyed one of the new tapes but did the backup on another- I also got error messages stating that there was a medium error and also excessive write errors and broken pipe????
Any thoughts??
