Intuity Audix R5.1 Software Load Problem

We have a problem occuring on a fresh install on an Intuity Audix. We are loading R5.1 from CD - clean installation - and the install goes perfectly per Avaya's written install specs until we get to the step for loading the various software packages. We select the main software selections per instructions and choose SWIN for Serial Integration and US ENG for announcements. When the software attempts in install these packages we get an immediate error that the High and Low Level installations fail for S3223RF&A and the remaining packages can't load so they fail also.

We are loading R5.1.28 off the CD. Unix loads perfectly and the base Intuity program software loads and we get to the main menu. When we select Software Admin and then load the remaining packages, this is when the errors occur.

It has a new hard drive which has passed all HD diagnostics tests along with all tests for the other components in the Intuity.

Any ideas on what would cause these errors? We have started from scratch a 2nd time and the same error occurs.

It is a velocity Pentium 200MHZ processor w/ (2) AYC 10 voice cards, RMB Card - Lan card, CDROM drive and it will be attached to a Magix switch.
