Intuity Audix LX release 2.1 Can not record name on any mailboxes

Asking for help here as I can not seem to figure this out, any help would be most appreciated, I recently had to replace a hard drive on an Intuity Audix rel 2.1, we did purchase from avaya, Installed the operating system and audix software from original install cd, everything is working great except that we can not seem to record any names on any mailbox, it says to press 1 to record name and press again after name is recorded, finally it say press the # key to accept name but gives us the message of name can not be recorded at this time. Confused here as when I press 1 it does play my recorded name but will not accept and get the above error message, any help would be most appreciated.
