My Exchange Server 2007 allows internal attachments to be upto 10mb but external ones don't go over 2Mb users get the error:

Requested #550 5.3.4 SMTPSEND.OverAdvertisedSize; message size exceeds fixed maximum size

Also we have a group of users who we don't want to send or recieve external emails unless they have been forwarded from another interal user, any suggestions?

Organization -> Hub Transport -> Global Settings -> Transport Settings. Do you have any limits defined there? That's what would determine the external mailflow message size limits.

On your other question: there are probably a bunch of ways to go about it. The first thing that comes to mind for me is to remove the SMTP addresses from their user object using the Recipient portion of the console, and uncheck the "Automatically update" box. If the user was somehow still able to send an outbound email, you could add a transport rule with a list of users to block outbound messages of.
