how to interprete A4, A5, A8, Af in below output? in SA38-0638-00 for 6c3 it is not mentioned.

Just an educated guess:

Looks like SCSI LUN A (10), followed by the drive's SCSI device number. The duplicates aren't, really, because they're on different adapters.
