Internal Callers getting a fast busy

Our media operator at extension 160 is telling me she gets a fast busy when trying to reach two other extensions being x875 and x303. The thing making me pull my hair out however is it seems to be at random. We use an old option 11c and the two extensions sit in the very same cabinet. They are on different line cards however but I wouldn't think that matters. Do you guys have any ideas or and experience with this?
 
You can try to move the two extensions on other ports on the same cards/different cards/different cabinet, if are available, or use spare parts on free slots to test if the problem persists.
 
If the two extensions are call forwarded to each other, it will produce a fast busy.
Next time trace both TN in LD80.
This happen all the time at my sites. We warn the users 1 time about checking with the person they are call forwarding to (so they don't create a loop). If it happens again we remove call forwarding.
I pretty sure normal hunting does not create a fast busy.
You can print the TNB for both phones it will show the number the phone was last call forwarded to.

You can set "Prevention of Reciprocal Call Forwarding" if that is in fact the problem. You won't have to warn them again if you set that

They did have a call forward loop setup. 875 forwarded to 535. 535 then sent to 303. 303 then sent to 875. Triple loop therefore broken.
