Internal Auto Dial On Phone Indicator Not Working

I have a MICS, (R1T1 I believe). The receptionist has an M7324 with several internal auto dial buttons. These buttons should indicate when a set is busy. The problem is sometimes a set is busy, but the indicator is not lit. Is anyone familiar with this issue? I've tried replacing the set and that did not help. It seems to be a random, but frequent occurance.

Seen this often. Not sure if it is a poorly seated software card or CO card or bad wiring. On one site I successfully reduced the occurrance by rather vigorously reseating the two CO cards. In fact it was the same R1T1 model you have.
