BCM I have a BCM where different extensions on different cards keep rebooting and coming back up. need some advice on what to look at

BCM50? 450? 200? 400? 1000?
What is the type of your machine and what version?

You may wan to look for a replacement power supply. I have had this issue with a few BCMs also

Same here...I had to change the power unit which didn't provide enough power to the modules 3&4

Check for the alarms listed below on this bulletin, depends on the system release. Each position of the Y-cable distribution on the BCM controls 2 bays of the media bay module, one leg is the top 2 and one leg is the bottom. If you have available space you could relocate it from top 2 to one of the bottom to rule out some items.

Business Communication Manager (BCM) 400 Y-Cable Field Maintenance Kit

Notice:
This bulletin replaces bulletin 2008008996.1. This update has
been issued to

Background:
Nortel reported an issue regarding Intermittent Media Bay Module failures per bulletin - 2006007338, entitled "Business
Communication Server (BCM): BCM400/200 & Expansion MBM Backplane Power Connector Issue: Maintenance
Solution"
This bulletin is issued to inform customers of the availability of the BCM Y-Cable Field Maintenance Kit as standard
merchandise item. The Y-Cable Field Maintenance Kit is specifically for the issue found on BCM400 units.
Please refer to bulletin 2006007338 for more information on the issue and solution for BCM200 units.

Analysis:
Per bulletin 2006007338, a problem found with the female MBM power connector that originated from the BCM power
supply, impacted a very small percentage of installed BCM400 and BCM200 base systems and BCM Expansion Units.
In the case of Digital Station Modules this is typically characterized by "247" errors, found in the BCM archlog. Other
alarms which may be generated if this issue is present include: Event 250 / Alarm:50, Event 251 / Alarm:51, and Event
252 / Alarm:52.
If a BCM system has this issue, system users can experience issues such as sporadic call drops, intermittent system
reboots, and frequent alarms being logged (like those mentioned above.) Resetting the BCM may clear the problem
temporarily. Symptoms would likely occur intermittently depending on vibration or system usage.
Nortel identified that the majority of the affected systems were manufactured in the second half of 2005, with the bulk of
them being in the fourth quarter of 2005.
As this bulletin is specific to the BCM400 product, the effected BCM Product Codes for the BCM400 are listed below.
Please refer to bulletin 2006007338 for BCM200 and Expansion Cabinet Product Codes.
The BCM 3.6 product codes potentially affected by this issue are:

Recommendations:
For BCM400 chassis, a Y cable is used to tap power from a connector not impacted by the pin that is subject to fatigue.
The procedure for identifying the affected connector(s) and performing field maintenance for the BCM400 chassis is
provided with the orderable BCM Y-Cable Field Maintenance Kit.
Nortel recommends that partners order BCM Y-Cable Field Maintenance Kits to service their regional geographic
requirements to enable fast resolution of system behaviors related to this issue.
For BCM400 system maintenance, order the: BCM Y-Cable Field Maintenance Kit, Part Number (PEC): NTAB3518, CPC:
N0125142
Please note that the Y cable is a no charge maintenance solution, i.e. the price for the cables has been set to zero. No
defective material needs to be returned to Nortel.
Required Actions:
If you have a system which shows the symptoms described in the section BACKGROUND, and the BCM product codes
match those provided in the section ANALYSIS, you should obtain the required parts for the solutions described above.
The BCM Y-Cable Field Maintenance Kit and BCM Power Supply FRU are available as standard BCM merchandise
products and can be ordered through the standard ordering process.
