Inserting a Check Box Form Field

I have Office2007, how do I insert a Check Box Form Field in Word? I installed the "Developer" toolbar, but now the check box is grayed out. I unprotected the document and still no luck.

Click on Developer|Controls|Legacy Controls. If the check box is greyed out here, you probably did an express install of Word instead of custom or full.
