Index Out of Date

I am, all of a sudden, getting frequent (i.e. several times a day) "Index out of date" errors in one of our key tables. This use to happen when we had frequent system crashes which has not been the case for some time and system crashes are not happening now. We have Paradox 9, a Windows 2003 Server and all the workstations (10) are Windows XP. Are there any suggestions as to what might be causing this problem?
 
First, LOCAL SHARE=TRUE must be set in *every* workstation BDE that has the BDE installed.

Second, Opportunistic Locking must be turned OFF on the server:

Third, IF the hard disk on the server allows, Write Cache must be turned OFF.

And just to make sure, if you have ANY Win98 machines, they must NOT connect to the data on an XP network. Corrupted Data WILL occur.
