InDesign CS2 Table of Contents not updating

I am using InDesign CS2's TOC tool for a TOC and an Index. I had to move the page order around and also basic mark up on a 106 pg catalog. The TOC is updating properly, but the index section updates only the 4 first and last spreads and leaves everything in the midddle out.

Any Suggestions?
