Incorrect caller ID being sent

I have an IP office 500 . The issue is we have one particular DID that is call forwarded, no physical phone xxx-923-8098. When someone calls that number,
xxx-210-2039 is the caller ID being sent, not the callers number,we have other DID setup to forward to cell phones as well. But the calling party caller ID is being sent correctly. Any ideas as to what is going on ?
 
Do a monitor trace to see what sent on the trunk to begin with so you know if it's the IPO och service provider sending the wrong number.
