Include footer and header when printing pdf web page

Printing a web page in pdf format.

Is there a way to include the browser's header and footer in the printout?

example:
I've tried all options shown in the "print" dialog.

If page is pdf, IE File |
>Page Setup is dimmed.
>Print Preview does not show.
 
Use File menu/create pdf/from web page.

On next window click Settings. On subsequent window there's a check box for headers and fotters.

What application are you referring to?
I don't know what OSX and G4 mean.
In IE or NN there is no File | Create PDF.

In the meantime I installed Acrobat 8; it behaves in same way.

 
App: Acrobat - not Reader

OSX/G4 = Mac with G4 processor - Acrobat works same in Windows and Mac.

I need to clarify something:
I stated that I have Acrobat 7; should have said Reader 7.

I downloaded the free product to print pdf web page files.
So when I do that, the main application is Internet Explorer.

So your suggestion "Use File menu/create pdf/from web page"
does not make sense. Because IE File menu does not have a "create pdf" option.

And I have Windows XP, so a Mac solution won't help.

evidently checks the users computer to see if Reader is installed. If it is, it auto-redirects to:

I needed to print the html version (with my bowser-set header and footer info) for a client so this is what I did:
>removed Adobe Reader application.
> (no redirect)
>printed OK

There may be a cookie that tells IE if I have Adobe Reader. I'm hoping I'll be able to erase that cookie when I need to print html webpages that auto-redirect to pdf page. Easier than removing the Reader application.

A person in another forum indicated the header and footer info is accessible if Acrobat ($$) is installed but not free Reader.
 
The information provided was about using full Acrobat, usually Acrobat Pro as this is the Acrobat forum. Acrobat Pro has the ability to simply access a web page and make a pdf without a browser.
