Inbound Name Display

Hello! I have a single DN that will not show the name display for inbound calls. Since this person has no phone, I have copied a working phantom set and routed it to the front desk operator, and then did a DCFW to voice mail. The operator sees a string of digits instead of the name.

The TN data is:
