Inbound DID calls mishandled

I have a Opt 11 c that runs very well with few problems except on occasion an inbound DID call is handed off to the wrong extension.
The user has not forwarded their phone to the extension the call ends up at. For instance, they could be calling extension 266 and receive extension 242 instead.
I can't find ANY issues as to why this is happening.
Has anyone had a similar experience and what was the cause?

Are you using BFS keys? I have seen those mis-used which can cause the problem you describe. If 242 has a BFS key of the TN for 266 and 242 presses that key, they will forward 266 to themselves. The user at 266 would never know it was forwarded.


 
If the extension is MARP'd on another phone appearance, which is consequently forwared, you would end up with the call going to the wrong place. Make sure the numbers are MARP'd on the owners set.
 
You say DID calls are going to the wrong extension.....If the same number is going consistantly (even though you said on occasion) have a look at the IDC tables in LD49
 
Are these DID's on a T1 or copper
