please see the below and tell me what it means:

>*LD 36
TRK000
.CALL 4 0 0 0
DN?
.19173592743

TN 004 0 00 07
.

when i called TN 4 0 0 0 i then put in my phone #. it rang on my phone's display as 212-490-3007, which is the phone # for TN 4 0 0 7, which the switch printed (see below my phone #). what's up with that??

If 212-490-3007 is the phone # for TN 4 0 0 7 then you are in the wrong switch, 212 is NYC.

what do you mean by I'm in the wrong switch? i was dialed in to my NYC PBX. 917 is a local area code in NYC as well. How come 4 0 0 0 gave me 212-490-3007, and then below that printed 4 0 0 7 (I didn't do that), which is the TN for 490-3007?

To find out the number of the trunk your calliing from call 800-314-4258

when do i call 800-314-4258? Do i do that thru the PBX? now you've got me totally confused, Ace!

Assuming 4 0 0 0 and 4 0 0 7 are analog COT trunks terminated to a universal trunk card, I'd say your carrier sends out 212-490-3007 as the ANI on all outbound calls over the trunks connected to these TNs.
