Identify program to patch

I am trying to use patch to apply changes to program source code. Patch file was created as a normal diff. The man page for patch says "The file to be patched may be specified on the command line or within the list of changes in a patch file." I want to do the latter, but don't seem to be able to get patch to recognize the name of the file I want to "patch" from anything I place within the patch file itself.

Does anybody know the secret for getting this to work? Or am I misunderstanding this altogether.
