ICA File Not Found - AFTER successful connection!!

This has me scratching my head big time.

We provide certain apps over a citrix connection, via an SSL appliance. This has been working perfectly for quite some time.

However, recently we've been getting the following...

SSL login - OK
Initial connection to citrix app or desktop - OK

then suddenly it will drop the connection to the citrix session, with the message "ICA File Not Found". It can be an immediate drop, or it can wait for up to 5 minutes. The weird thing is it DOES connect, before it says it can't find that file...

is it one client machine, or a number of them? what verison of IE are the client running? can they be upgraded? Have the Temporary internet files been deleted on any of the troublesome machines? What version of the citrix client?

We had this issue where the Secure gateway had set a timout for the ticket to expire (ica file) and upon launch it would delete itself, only later to have IE or the clitrix client call the same thing and then of course "not find" the same file as it had already been deleted?
