IC Error - Internal Error. ICCOST-FETCH (2741 ICADED1.C)

As noted in the subject I'm getting an error when trying to import IC adjustments:

Description: Internal error. iccost-fetch (2754 ICADED1.C). Write down the message, leave ACCPAC and Windows and return to the system prompt. Reset your computer and try the task again. See Appendix A in the I/C User Guide.

Obviously the reboot did nothing and I don't believe there are any issues with the files. I don't see anything related to this error on the forum so I'm wondering if anyone has encountered this before?

One of our controllers has import the item list and is now trying to make adjustments but this is the error he gets. Nothing else has been done.

Service packs?
Data integrity check OK?
Does a manually entered adjustment work?
 
5.5 and 5.6 (I copied the DB locally and upgraded and get the same issue).

Manual adjustment works and so does a very basic import which leads me to believe something in the file is incorrect.

Unfortunately this controller is at a remote site and trying to get this done now so hopefully someone knows the error or I figure it out before he leaves.
 
An error like that means something basic is wrong with the format of the import file. Export a manually entered batch and compare.
