I have an old IBM Deskstar 40g HDD that makes a rather strange double squeeking noise every now and again. Sometimes this happens every day, sometimes twice a week.

Scandisk reports no problems with the drive at all (full scan).

I have had this drive for at least 4 years and it has been doing this since I got it. I don't keep any important data on it as I don't trust it!

Any ideas as to what is happening?

Early IBM deskstar's have the Deathstar knickname for good reason.
I believe the GXP60 and GXP75 series, capacities upto 80gig? within a certain manufacturing time frame, had unusually high failure rates.

Note* these were the only models/capacities effected unfortunately newer versions/models undeservabley continue to carry the stigma of the earlier model failures.

True. I didn't know it was just those models it effected. I knew it was just the older variants of the drive.

That's why I got jumpy when this one started with the squeeking!

Any ideas as to what it is thats happening to it? Someone told me that it was prob the drive failing, but it's been up 24/7 for the last three years.

Listen I'd agree that the drive is due to fail soon. Abnormal noises are a bad sign. Better just get a new one - unless you totaly don't care about losing your stuff (if you only keep some games and old programs on it or somesuch).
