I can't access a custom form in outlook

I am using Exchange Server 5.5 and for some reason on one workstation I can not access a custom form in Outlook 2000. It is a Windows NT workstation. The error states that Outlook will use an Outlook fcorm instead. Any help would be appreciated. thanks
