I/C Post Inventory Reconcilition Error message

We have received an error message in one of our companies it states the following. It occurred during the posting of I/C Post Inventory Reconciliation. It is holding up completeing the month end process.

1 E Adjustment Header. Record already exists.[Microsoft][ODBC SQL Server Driver][SQL Server]Cannot insert duplicate key row in object 'ICADEH' with unique index 'KEY_3'.
2 E Internal error. postw-finish-adj-hdr (1618 ICPOSTW1.C). Write down the message, leave ACCPAC and Windows and return to the system prompt. Reset your computer and try the task again.

Can anyone provide what this means and what needs to be done to correct it. Any comments or suggestions will be much appreciated.
