First, test to see if the .rhosts file is being read. Make a test .rhosts file with a single line with a "+". This will allow access to anyone temporarily. If you can log in without a password, then the file is being read and there is some inconsistency with the name <-> ip conversions.
Things to look for:
1) One or both of the machines may also be known by an alias. If this is the case, make sure that all possible machine names are included in the .rhosts file.
2) The machines may know each other by fully qualified names, for example: machinename.domainname.com, if this is the case, the .rhosts file must contain the fully qualified name.
For troubleshooting purposes, run a snoop and look at the names by which the machines are communicating.
