Hunt Pilot Ringing Busy

Our location has the main line ringing into a Hunt Pilot which broadcasts to the group. We run a local PRI at this site. I am getting constant complaints that the line is busy. I have verified that at the time of one of the complaints no lines were in use.

I am wondering if my Hunt Pilot (32500) is configured wrong.
couple things I question:
1. My Hunt Pilot is se to *32500 for destination with a CSS of site internal local LD DN CallFwd - I have seen other sites setup with xlation inside only.
2. My Line group consists of only one DN in the line group members. this DN is placed on each of the phones on line 2 at this site. I also broadcast the distribution algorithm.

Any idea's what may be causing this busy signal??? I did have the circuit tested and all 23 channels tested clean.
 
when is the busy happening? When you dial the *32500 from another ip phone or when dialing the DID assigned to it?
I would bet it is the latter and that is a translation/css issue.
 
Funny you ask. Actually the latter but I have had some cases with other sites where I do get a busy signal when dialing the hunt pilot.
I am thinking that I am missing something with the Hunt Pilot setup or setting something wrong. Pin pointing it is the issue!
I appreciate any help in isolating and troubleshooting this problem. Reading the Cisco website makes my head hurt :)
 
Here is a question. Do I need to have a translation pattern created for the sites main line to translate to the extension assigned for the main line. Currently I do not, I only have the the hunt pilot setup.
