I have to put a 9150 in Rome and connect it to a 11C in Milan, there are 15 users in Rome, they need continual connectivity to Milan for voicemail and PSTN access , what can I install as a back-up if the main link fails, if the link goes down, any cheap and cheerful ideas ?

Stick it over the open internet and NAT the endpoints? IP cost only
 
Have a ISDN BRI installed at each location. If the network is degraded or quits the 9150 will switch over to BRI without the subscriber ever knowing it.
 
unkalary is on the money, we have the same situation here and it (BRI between points)works great.
