How to undo update that makes server unable to boot

I have not encountered this yet but am concerned it could happen when there is no real way of testing before installing patches and windows updates on a server. Has anyone encountered a situation where a Windows Server 2003 computer does not boot up after installing a Microsoft patch or update through automatic updates or via WSUS (or even through Shavlik HFNetchk)? I would think some action via the Recovery console is required but any suggestions are aappreciated.
