How to protect check box in PDF form?

Here's what I wanted to do. We use a PDF file for our time sheet. After we fill in the form, the time column will total up and show the amount of hours worked during that week. Now, before this form can be accepted by the HR department, our supervisor has to sign it. So what I like to do is scan the supervisor's signature in and then put a check box or readio button next to the supervisor's signature. So when the supervisor click the radio button or the check box to approve the time sheet, a window will popup for the supervisor's password to varify that the person approving the time sheet is really the supervisor and not someone else. Can this be done in Acrobat? Basically what I want is to somehow protect the radio button or the check box so a window will popup for the password, unless there is an easier way to do this in Acrobat.
