There isn't a way to have the modifier set the item price to $0. That's not entirely true, but you'll want to use a promotion to set the price to 0 as it's better for reporting.
You can still add a Birthday modifier that will print in the kitchen using a Modifier Code.
I would use promos or comps for this however in some instances a zero priced item is beneficial. (rent is based on gross sales, sales tax is calculated on pre-discount sales etc.) A modifier can carry a negative amount so if you have an dessert for $5 and a modifier attached for -$5 the end result should be $0. Of course you have no security as the modifier is readily available to all order entry employees. You can also have a void type created for birthday if you are trying to accomplish what is listed in the ( ) above and want to have manager control over it.
