How to insert an Adobe Photoshop (PDF) image into a form

I am following all of the help instructions to insert an OLE object into my form, however, I keep getting the icon instead of the actual image. I have checked all of the property settings and everything seems set for image content. I don't know what else to do, does anyone have any suggestions????
