HOW TO GET A FORM TO FILL WITH DATA FROM TABLE

I am trying to get a form to fill in data in columns that I setup in the form. Typically once you choose from a list box a phone number or customer name, the other data columns which are linked to the customer or phone would fill in automatically. Is that not possible with this software??
