How to create a customized toolbar?

Please somebody helps me.

I have some menu items in the application. If a user wants to choose some menu items then create a new toolbar, how can I program it? I am using Visual C++ 6.

I don't want to do the fancy drag-n-drop things. I just want the user to choose menu items from a list. So the question is how to create and control a dynamic toolbar.
