how to add new drive to fail RAID 5 array

I had 3HD in RAID5 - CPQ DL580 Smart Array 5300 (Port 1, drive 0,1,2), but for testing I moved this disks to server with Smart Array 3200. Now I removed this disk to they orginal possision, and I notice that SA 3200 label one of disk on port 2 drive 4. Smart Array 5300 read configuration from disks and put RAID5 in fail state (there is only Port 1 in this array). The OS was able to start, but is working only on 2 disks. The problem is how to remove this disk labeled port4 and add new disk do the array, or how to change configuration on this disk back to port 1.
