How to add a template to forms wizard

Does anyone know how to add a template to the forms wizard selection of "what style do you want?"

First step is to create the table "UsysReginfo". The second step is to checkout Microsoft's website. I don't mean to be sarcastic, but they explain what fields you need in the table and what values they may contain. I've created my own wizards for forms, comboboxes, listboxes, and textboxes. Each definition is different.

If you can't find it on Microsoft's website, use a search engine to search for UsysReginfo. Might be a listing on this site.
