Yes, you can create your own wizard. You'll need to create the code (forms, etc) to programmatically create the report. And then reference your "Wizard" db via the Add In Manager. It also involves creating a UsysRegInfo table.
However, if you have a report template you might check out the AutoFormat feature. Just open a report in design view, select Format from the menu bar, then select Autoformat... It will explain how to add you template to the list.

for your second propose, I have to say that autoformat is not enough for this matter because it just make any new report label as my sample report not completely asme??
