How do you set up your own output size in the printer driver?

I am having problems with regard to the document size differing from the paper size. I am creating documents in quark and using distiller to create a pdf document. Everytime I do this there is always a slight difference eg I have created a document which is (w) 148 x 210 (h) but the paper size reads 147.81 x 209.9. I have been informed that if I set up my own output size in the printer driver then I can overcome this problem however I do not how I do this. Can anyone help?

This will have no effect on your output, it will output correctly at repro.

Acrobats default measurement unit is in inches but converts to the nearest MM. Bottom line is...its not a problem.
