How do you create a time/date stamp Macro for Outlook 2000?

I am looking for a macro that will insert a time/date stamp as well as the user's name stamp when the user adds comments to an individual contact.

Reason for macro: My company has public contacts, and would like to add on-going comments under each contact, but have each comment stamped with time/date/user's name.

Please advise.
